problem with the questionnaire

Hello! There is a serious problem with the questionnaire that my clients fill out. All the questions are on top of each other and this makes it impossible to read and complete it. Apparently it's been a few days since I haven't had any notifications since then. I ask for the fastest possible feedback. Thanks!

    Hi all. The issue has been fixed!

    You may have to clear your cache in order for this to take effect if you’re still seeing overlapping text.
